UPDATE- OFFICE CLOSURE NOTICE DUE TO WINTERY WEATHER CONDITIONS NOTICE: All FPHSA Outpatient Clinics and Offices will be closed on Tuesday, January 16, 2024, due to the extreme winter weather conditions. Please note that we made efforts earlier in the day to convert all “in-person” appointments to “virtual” appointments; however, office closure has been declared for all statewide offices and we feel this is in the best interests for the individuals we serve and our staff.
