FPHSA NEWS

Please Take our Brief Behavioral Health Community Survey

Your Voice Matters!  FPHSA is conducting a brief behavioral health survey throughout Region 9 to better understand where we shine and where we have room for improvement. It is important that your local community is represented. All responses are completely anonymous. Your honest feedback is greatly appreciated! Thank you in advance.

Please feel free to share this with family, friends, co-workers, your community, and your entire LISTSERV if you are a provider, as the more information we have, the better we can serve our communities! Thank you in advance for your participation.

Please submit the survey no later than Tuesday, March 12, 2024.
