Office Closure: Hurricane Francine

Due to Hurricane Francine, all FPHSA outpatient clinics and Administrative offices will be closed on Wednesday, September 11 through noon on Thursday, September 12.

If you experience a behavioral health crisis during office closure, you can access our after-hours emergency line through ViaLink by dialing ‪1-855-268-1091 or “211”. You can also access crisis chat at ‪www.vialink.org. Text Chat is also available by texting “TXT211” to ‪504-777-EASE (3273).

If you have a medical emergency, please go to the nearest Emergency Room or dial 911.

Single Point Of Entry

The Entry process determines the applicant’s eligibility for supports and services in the Developmental Disabilities service system. Meeting criteria for participation in the system does not ensure that a person is eligible for specific supports and services. Anyone can apply for services for themselves or another person. However, the individual must agree in writing to participate in the process.

To initiate the process of determining eligibility for services, please call (985) 543-4730 anytime Monday through Friday from 8:00 a.m. to 4:30 p.m. and request to schedule an entry appointment for Developmental Disabilities.

EarlySteps children may apply either through their Support Coordinator or by calling the FPHSA office and requesting Entry. The Entry process can begin when the child is two (2) years and six (6) months. From age 0 to age 3, the IFSP will serve as the approval in the system. In order to keep the EarlySteps date of entry called the “Protected Date” a child must finish the entry determination by age five (5).
