WINTER WEATHER CONDITIONS NOTICE REGARDING SERVICES ON WEDNESDAY, JANUARY 17

All FPHSA Outpatient Clinics and Offices will be closed for “in-person” services on Wednesday, January 17, 2024, due to the extreme winter weather conditions; however, all in-person appointments that are scheduled will be switched over to a virtual appointment instead. Please expect a call providing you with the information you need to connect with us. At the time of your appointment, please be sure to accept any calls that may appear as a blocked number. (Please also update your phone settings to allow calls from blocked numbers) We thank you for your cooperation and understanding as we take the necessary safety precautions to protect you and our staff.

If you or a loved one experiences a behavioral health crisis during this time, you can access our after-hours/ emergency line by dialing 1-855-268-1091 or “211”. You may also access crisis chat at www.vialink.org. Text Chat is also available by texting “TXT211” to 504-777-EASE (3273).

If you have a medical emergency, please go to the nearest Emergency Room or dial 911. The National Suicide and Crisis Lifeline is 988.

Referring Providers

If you would like to refer someone to Florida Parishes Human Services Authority’s residential treatment programs, Alcohol and Drug Unit (ADU) for women, or Fontainebleau Treatment Center (FTC) for men, please email the supporting please fax the listed information below to 985-624-4123 for consideration. 

  1. A completed statewide referral form (click here);
  2. The most Recent Psychosocial Assessment;
  3. MAR (if hospitalized); and
  4. Most Recent Progress Notes (indicating mental/physical stability with expected discharge date).
